I have difficulty swallowing foods and fluids anyone know something?

Im 22 years of age. For a few years now I have had difficulty swallowing fluids and foods. It has gotten worse and has caused me to gag and puke my food back up. Its very hard to swallow sometimes. I eat slowly... usually the last one to finish her meal. I have not been to a doc for it and dont have insurance anyways... the clinic here sucks and pretty much cater to those looking for pills to get high... Does any body have any info they can give me other than what you can google... cause geez i can do that myself... i just cant never find a specific... i get sore throats quite often reccently and i am a smoker... lol kinda asking for it i guess... i will be quitting just not right now...
